Introduction to BSL Course Application Form

As part of providing our services, MDC needs to collect and store certain personal information from you. MDC understands the importance and sensitivity of your personal information. MDC will treat your personal information with the utmost respect and confidentiality. However, there may be limited occasions when we need to transfer your medical/personal information to people who need to know, for example, in relation to planned activities or medical emergencies.

Please note that the above course is subject to a minimum enrolment requirement. We will proceed with the course only if we receive applications from a sufficient number of participants. If this requirement is not met, the course may be cancelled or rescheduled, and all applicants will be notified accordingly. Any application fees paid will be refunded in the event of course cancellation due to insufficient enrolment.

Please note: Invoices will be sent via email only when you have received a confirmation of your place on the course.
